I understand that only an applicant or the applicant’s authorized agent can make an enrollment (move-in or switch) for service. I certify that I am eighteen (18) years old or older, and that for the above listed address, I am legally authorized to select or change a retail electric provider (REP). I authorize that the REP, Budget Power, may conduct a credit check in accordance with the Fair Trade Act and, take the necessary steps to become the REP for the address listed above by switching, establishing, or changing my electric service. I also understand that if my enrollment involves a switch request, I have a time period of three (3) federal business days to review my terms of service with Budget Power and to rescind it without penalty. After the three (3) federal business day time period has ended, I understand that I may be subject to pay an early termination fee if I cancel my agreement before the end of my contracted term. I have read and understand all of the authorized disclosures, the Terms of Service (TOS), Electricity Facts Label (EFL), and Your Rights As A Customer (YRAC) documents for my chosen product. I understand that I will receive a written copy of the TOS document that explains all of the terms of the agreement and how to exercise the right of recission via email, or I can contact Budget Power to request the information be sent in the mail.
